Health Forms & Info
All students must be up-to-date with physical examination and immunization requirements. All completed forms must be turned into the School Health Office before October 15.
Physical Examination Forms
- All students entering Early Childhood/Preschool, Kindergarten, 6th grade, and 9th grade are required to have a current physical examination completed via the approved Illinois Department of Public Health Form. These are requested prior to the first day of attendance but required by October 15th.
- Parents/guardians must complete & sign the Health History section on page 2 of the physical exam report.
- An IHSA / IESA sports physical does not fulfill the requirement
- Reports may be dated no earlier than one calendar year prior to the first day of attendance.
Immunization Requirements
- All students must be up-to-date with the State of Illinois immunization requirements.
- All students entering 6th, 7th, 8th, 9th, 10th, and 11th grade must show proof of receiving one dose of Meningococcal vaccine (meningitis).
- All students entering 12th grade must show proof of receiving one dose of Meningococcal vaccine after the age of 16.
- All students entering grades six through twelve must show proof of receiving one dose of the TDAP vaccine.
